President Donald Trump is facing renewed scrutiny over his neurological fitness after a prominent NBC News physician warned that the 79-year-old is displaying 'cascading' signs of cognitive deterioration.
Dr Vin Gupta, a practising public health physician and medical analyst, pointed to Trump's erratic performance at the 2026 World Economic Forum in Davos as evidence of worsening 'word-finding difficulties' and 'tangential speech'.
During a high-profile keynote on Wednesday, the President repeatedly confused Greenland with Iceland—two distinct nations—while threatening NATO allies over his stalled bid to acquire the Danish territory.
While the White House has dismissed the concerns as 'blatant lies,' the rare public intervention by a medical expert has reignited a national debate over the mental acuity of the oldest sitting president in American history.
Speaking to MeidasTouch, Dr Gupta outlined a cascade of concerning symptoms that he says warrant urgent professional evaluation. 'He's the president of the United States, so let's hope, beyond the headlines, that he is getting the care that he needs,' Dr Gupta said.
The doctor's concerns have been amplified by the president's recent behaviour on the world stage. Trump shared private text messages from global leaders on social media and delivered a 105-minute White House speech that observers described as 'incoherent' and 'disoriented'. These incidents are the latest in a series raising questions about his fitness for office.
The Symptoms Of Cognitive Decline
Dr Gupta has identified a troubling list of symptoms that he believes warrant serious medical scrutiny. He notes that Trump displays 'word finding difficulties' and a marked inability to maintain focus during lengthy speeches. 'The fact that he has inattention, he loses his train of thought. Everybody is describing these speeches as meandering, covering 30 different topics in the course of, say, two minutes,' Dr Gupta explained.
The physician highlighted additional concerns, including clear difficulty expressing thoughts and completing sentences. 'Tangential speech,' he said, 'of course, memory issues. He doesn't know that he's talking about Greenland for even the course of a sentence and confuses it with Iceland.' According to Dr Gupta, such symptoms create 'significant concern' about the president's cognitive health and suggest a 'cascading' pattern.
What makes the assessment particularly compelling is Dr Gupta's observation that these issues appear to be worsening. The doctor pointed to Trump's recent private message to Norway's prime minister, Jonas Gahr Støre, as evidence of a broader deterioration in judgment. In that message, Trump linked his aggressive stance on Greenland to the Nobel Peace Prize decision, a connection that struck Dr Gupta as alarming.
'The fact that he would even put a letter like that out there in the public domain has caused a lot of alarm. Who does that? That is not even behaviour for him that we think would meet a standard of decorum or just proper behaviour,' Dr Gupta said.
Family History And Clinical Correlations
Trump's family medical history adds another layer to the concern. His father, Fred Trump Sr, was diagnosed with Alzheimer's disease in his eighties, a detail that Dr Gupta says suggests a genetic predisposition. 'We're observing the trends that we first noticed a few months ago. There's a symmetry between some of these earlier signs in his case and what his father went through,' Dr Gupta explained.
These observations have found support elsewhere. Mary Trump, the president's niece, has also raised alarm bells. 'I think that Donald has had undiagnosed, untreated, psychiatric disorders for many, many years, predating his first administration,' Mary told CNN's Erin Burnett on Thursday.
When asked whether the symptoms align with early Alzheimer's disease, Dr Gupta was circumspect but frank. 'A lot of the symptoms that we just talked about are consistent with those diagnoses.' He also raised the possibility of frontotemporal dementia, noting that 'the acting out, behaviour changes, lashing out, acting unreasonable. One could say this is his core fundamental personality based on what we've seen over, frankly, his entire adult life, but it's getting worse.'
White House Pushback: 'The Sharpest President'
The White House has launched a fierce counter-offensive against claims of decline. On 22 January, spokesperson Liz Huston released a statement defending the President's stamina and mental sharpness.
'President Trump is the sharpest, most accessible, and energetic president in modern American history. These claims from disgraced doctors are nothing more than partisan attacks and blatant lies,' said Liz Huston.
To bolster this claim, Trump posted on Truth Social that he recently 'aced' the Montreal Cognitive Assessment (MoCA) for the third time in a year, though medical experts noted that the screening tool is designed to detect severe impairment rather than subtle executive dysfunction.
