Donald Trump's 'Strange Bum Bulge' Prompts Commentators To Speculate It 'Looks Like a Book'
A strange shape beneath the president's jacket has generated plenty of theories but very few verified answers

President Donald Trump's mystery jacket bulge drew fresh attention after White House aide Margo Martin shared footage from Charlotte, North Carolina, on Wednesday, 16 September. The clip showed a box-like shape beneath the back of Trump's suit jacket as he spoke to reporters near Air Force One.
The Daily Beast reported the sighting the next day and said a similar unexplained shape had appeared earlier in the year. No public evidence has established what caused the bulge or linked it to any medical condition.
The latest attention comes against a backdrop of scrutiny over Trump's publicly disclosed health issues. His lower-leg swelling and hand bruising have been addressed by the White House physician, but those records do not identify the object beneath his jacket.
Donald Trump Bulge Draws a 'Book' Comparison
The Daily Beast said Martin's Instagram Story captured the latest appearance of the shape during Trump's North Carolina visit. A White House video independently confirms the president spoke to reporters upon arriving in Charlotte on 16 September.
Two days later, Joanna Coles and Hugh Dougherty discussed the footage on The Daily Beast Podcast. Coles is the outlet's chief creative and content officer, while Dougherty is executive editor.
Dougherty said, 'It looks like there's a book under the back of his jacket on his right-hand side.' Coles asked whether the shape could be a medical device, then joked about the possibility that Trump was carrying a book.
Neither editor claimed to know what the object was. Dougherty later called the bulge 'a mystery', making clear that its identity had not been established.
That distinction matters. The footage shows an exterior shape through Trump's clothing, but it does not reveal what caused it. Unsupported theories have circulated online, including medical speculation, without evidence confirming any of them.
The Daily Beast said it asked the White House to explain what was visible beneath Trump's jacket. White House spokesman Davis Ingle did not identify the object and instead rejected the outlet's premise.
Ingle accused the publication of promoting 'baseless conspiracy theories' and defended Trump's health, energy and work schedule. The response challenged the speculation but did not answer what caused the visible shape.
Donald Trump's Medical Records Offer Separate Context
Trump's health records provide relevant context, though they do not explain the bulge. In July 2025, Physician to the President Capt. Sean P. Barbabella said Trump had chronic venous insufficiency after reporting mild swelling in his lower legs.
Barbabella described the condition as benign and common, particularly among people over 70. He said testing found no evidence of deep vein thrombosis, arterial disease, heart failure, renal impairment or systemic illness.
The same memorandum addressed bruising seen on the back of Trump's hand. Barbabella said it was consistent with minor soft-tissue irritation from frequent handshaking and aspirin use.
There is no evidence in that medical update linking either condition to the shape beneath Trump's jacket.
Trump later underwent his annual medical examination at Walter Reed National Military Medical Center on 26 May 2026. Barbabella's report was released on 29 May and recorded normal neurological findings.
The physician reported normal mental status, cranial nerves, motor strength, sensation, reflexes, gait and balance. He also said Trump scored 30 out of 30 on the Montreal Cognitive Assessment.

Barbabella wrote that Trump remained in 'excellent health' and was 'fully fit' to perform presidential duties. Those were the physician's conclusions from the examination, not an explanation for the jacket bulge.
Coles referred to Trump's leg swelling and hand bruising while discussing the footage. Those conditions have documented medical explanations, while the unidentified shape does not.
For now, the strongest verifiable description is also the simplest one. The footage shows an unexplained, box-like bulge beneath Trump's jacket, and no primary source has publicly identified what it was.
