Hayden Panettiere Died From Fentanyl and Other Drugs, Coroner Rules Death an Accident

Hayden Panettiere died from the toxic effects of fentanyl and several other drugs, with the Greenville County Coroner's Office ruling her death an accident.

The actress, best known for her roles in 'Heroes' and 'Nashville', was 36 when she died on 16 August 2026 at a residence in Greenville, South Carolina, after being found unresponsive and could not be revived. Her death remained under investigation for more than a month while authorities awaited toxicology results.

According to the coroner's findings reported on Tuesday, 22 September, Panettiere's cause of death was the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. The manner of death was classified as an accident.

Hayden Panettiere's Cause of Death Revealed

The toxicology findings provide the first official explanation for Panettiere's sudden death. Fentanyl is a powerful synthetic opioid, while alprazolam is commonly known by the brand name Xanax. Methocarbamol is a muscle relaxant, and quetiapine is a prescription medication. 4-ANPP is a chemical associated with fentanyl production.

Authorities had previously cautioned against reports about the cause of death while the investigation was continuing.

On 16 September, Panettiere's representative said toxicology results were still pending and urged the public to wait for official findings rather than rely on unverified reports. The coroner's ruling now establishes the official cause and manner of death.

Panettiere Was Found Unresponsive in August

Panettiere was found unresponsive at the Greenville residence on the afternoon of 16 August. The Greenville County Coroner's Office said a 911 call reporting a cardiac arrest was received at approximately 1.51pm.

Emergency medical personnel arrived and began advanced life-support measures, but efforts to resuscitate her were unsuccessful. She was pronounced dead at approximately 2.32pm.

An autopsy was carried out shortly afterwards. The initial examination did not establish the cause of death, leaving toxicology testing and other investigations to determine what had happened. Police had also said there was no indication of foul play.

Panettiere's death remained an active investigation involving local authorities and, according to previous reporting, federal authorities.

Her Final Days Remained Under Investigation

Panettiere's death drew attention because of the sudden circumstances and uncertainty surrounding what had happened inside the Greenville property.

A police report said officers spoke with Panettiere's former boyfriend Brian Hickerson and his brother Zach Hickerson at the scene. The report said Brian showed officers a bag of medication that Panettiere had been taking.

Earlier reports also said Narcan, a medication used to reverse opioid overdoses, was found at the residence. However, authorities had not publicly determined whether drugs played a role until the toxicology findings were completed.

It was previously reported that federal authorities were examining where drugs and medications potentially connected to Panettiere's death came from. That investigation was separate from the coroner's determination of her cause and manner of death.

Remembering Hayden Panettiere

Panettiere began working as a child actor before becoming internationally recognised through television. She gained widespread attention for playing cheerleader Claire Bennet in 'Heroes', which ran from 2006 to 2010.

She later starred as Juliette Barnes in the country music drama 'Nashville', appearing throughout its six-season run. Her film credits included 'Remember the Titans', 'Racing Stripes', 'I Love You, Beth Cooper' and 'Scream 4'.

Panettiere had also spoken publicly about struggles with addiction and mental health. Her personal experiences became part of the story of her later life, including her decision to seek treatment during her time on 'Nashville'. Her death came just days before what would have been her 37th birthday.

The coroner's ruling provides an official explanation for her death, confirming that the combination of fentanyl and the other substances found in her system caused the fatal toxic effects and that authorities have classified the death as accidental.
