Fresh online speculation surrounds Donald Trump's legs after new video footage, even as his physician reports improvement in lower-leg swelling and confirms excellent health By The White House, Public Domain,/wikimedia commons

Fresh questions have surfaced about Donald Trump's health after a video of the president walking outside the White House appeared to show raised outlines beneath his trousers, reigniting online speculation even as his physician has noted improvement in lower-leg swelling.

The footage, which began circulating widely on the social-media platform X in late September 2026, has prompted some users to question whether the 80-year-old leader has an undisclosed health problem, although there is no verified evidence from the footage that he does.

The White House physician said in May that Trump remained in 'excellent health', with strong cardiac, pulmonary, neurological and overall physical function.

What the Footage Shows

The latest stir began with clips showing Trump descending steps and walking through a corridor alongside aides and Secret Service agents.

Viewers spotted two symmetrical, raised outlines beneath the fabric at the back of his calves, sparking theories ranging from compression garments to ankle-foot orthoses, or AFOs, which are braces used to support the foot and ankle. There is no verified evidence that Trump was wearing either.

Some social-media users offered detailed speculation about what might be causing the outlines. One X user asked whether Trump was wearing 'leg braces', while another suggested they could be 'bespoke AFOs with Velcro straps around the top just below the knee'.

Others were not convinced something medical was at play and offered alternative explanations for the president's gait and posture.

Trump's medical team has not disclosed that he wears a brace, orthotic or other assistive device, and there is no verified information identifying the outlines beneath his trousers.

Photographs and short video clips cannot establish a medical diagnosis, and medical experts have cautioned against drawing conclusions from brief visual evidence alone.

Trump's Documented Leg Condition

Trump's leg concerns are not new. In July 2025, visible swelling in his ankles led to an ultrasound that diagnosed chronic venous insufficiency, a condition in which the veins have difficulty returning blood from the legs to the heart.

The White House described the diagnosis as benign and common among people over 70, with testing finding no deep vein thrombosis, arterial disease or heart failure.

Chronic venous insufficiency affects approximately one adult in 20 and occurs when the valves in leg veins become damaged or weakened, allowing blood to pool in the lower extremities rather than flowing efficiently back toward the heart.

Common symptoms include swelling in the ankles and lower legs, visible varicose veins, skin discolouration and, in more severe cases, leg ulcers. Treatment options range from compression stockings and leg elevation to more invasive procedures such as vein ablation or surgery.

Then-White House press secretary Karoline Leavitt read from the physician's statement in July 2025, characterising chronic venous insufficiency as a 'benign and common condition' among older adults.

The statement emphasised that comprehensive vascular testing had ruled out more serious conditions, including deep vein thrombosis, arterial disease and signs of heart failure.

May 2026 Annual Physical Examination

Trump's most recent annual physical examination took place on May 26, 2026, at Walter Reed National Military Medical Center in Bethesda, Maryland. In a three-page memo released by the White House, US Navy Capt.

Sean Barbabella wrote that Trump 'remains in excellent health, demonstrating strong cardiac, pulmonary, neurological, and overall physical function'. The report noted 'slight lower leg swelling was noted, with improvement from last year'.

The comprehensive examination involved 22 medical specialists and included cardiovascular, neurological, dermatological and other assessments. Trump underwent a CT scan and other heart imaging, along with cancer screenings and other preventive assessments. According to reports, the visit lasted approximately three and a half hours.

After the examination, Trump wrote on his Truth Social platform, 'Just finished my 6 month physical at Walter Reed Military Medical Center. Everything checked out PERFECTLY.' While Trump characterised it as a '6 month physical', the physician's memorandum describes it as his annual medical examination.

The physician's report did not identify an abnormality affecting Trump's gait or neurological function, although it did note slight lower-leg swelling that had improved from the previous year. The report recorded normal findings in several areas, including mental status, gait and balance, while also noting slight lower-leg swelling.

Online Speculation About Neurological Conditions

Some social-media users have also speculated about whether the way Trump was walking could indicate a neurological condition. There is no evidence from the footage to establish such a diagnosis, and medical professionals have emphasised that brief video clips cannot substitute for comprehensive clinical evaluation.

His May 2026 medical report said his neurological examination was normal, including his mental status, strength, sensation, reflexes, gait and balance.

The Alzheimer's Society notes that some forms of dementia, including dementia with Lewy bodies, can cause movement problems such as stooped posture, shuffling and balance difficulties.

However, that general medical information does not establish that Trump's movements in the footage are related to dementia, and the physician's report explicitly documented normal neurological function.

What Remains Unverified

The cause of the outlines seen beneath Trump's trousers has not been independently verified. What is documented is that Trump was diagnosed with chronic venous insufficiency in July 2025 and that his physician noted slight lower-leg swelling in May 2026, with improvement from the previous year.

Medical experts have cautioned that visual observation alone cannot determine whether someone is wearing compression garments, orthotic devices or other assistive equipment. Tailoring, fabric thickness, lighting conditions and camera angles can all affect how clothing appears in photographs and video footage.

For now, the cause of the outlines remains unclear. Trump's legs are likely to continue attracting attention from observers and social-media users, but without official disclosure from the White House medical team or independent verification from qualified medical professionals, any conclusions about what the outlines represent remain speculative.
